Dear All,
Please use the following check list before submitting your application/ renewal form of SAIL Mediclaim Scheme. Please forward this mail to all your members and may send such e-mail ids to fed.sailex@gmail.com for record
- Application form- filled up and signed
- Printed copy of Payment Receipt
- Cancelled cheque
- Two photographs for each Employee and the spouse- One each to be pasted on the Form and second copy to be stapled on the top of the form
- Xerox copy of your and your spouse's I-Card
